Exams take approximately 20 minutes, and we ask that you arrive 15 minutes early to ensure a smooth check-in process. Be sure to review our Physical Exam Checklist before scheduling.

DOT Physical 
Exam Checklist

Everyone needs to:
  • Bring a state-issued photo ID. Best option is your driver’s license.
  • Be prepared to provide a urine sample to test for kidney functions and protein levels
You may also need to bring:
  • Corrective lenses (glasses or contacts), if you use them
  • Hearing aids, if applicable
  • A complete medication list (paper or electronic), including:
    • Medication Names*
    • Prescribing provider’s name, address, and phone number
  • *Some prescriptions may require a DOT Medication Clearance Form (MCSA-5895) completed by your prescribing provider. This is commonly required for medications such as: Controlled substances (Schedule II-V), Opioid pain medications, Benzodiazepines (e.g., alprazolam/Xanax, lorazepam/Ativan, clonazepam), Sleep medications (e.g., zolpidem/Ambien), Muscle relaxants, ADHD medications (stimulants or non-stimulants), Certain antidepressants, antipsychotics, or mood stabilizers, Anti-seizure medications

*If you are unsure whether your medication requires clearance, it is best to bring a completed MCSA-5895 to avoid delays.

Additional paperwork may be needed for some conditions as noted below:
  • Diabetes
  • Heart or cardiovascular conditions (past or present)
    • Relevant testing such as EKG, stress test, echocardiogram
    • And/or a letter from your cardiologist stating your condition is stable and does not interfere with safe driving
  • Sleep apnea / CPAP use
    • CPAP compliance documentation for at least 90 days, which can be shown by:
      • A printed compliance report
      • Bringing your CPAP SD Card
      • Showing compliance history on your CPAP phone app
  • Recent injury or surgery
    • A release-to-work letter from your treating provider

Please make sure you do not:
  • Chew or use tobacco directly before or during the exam
  • Drink excessive beverages containing caffeine before the exam
  • Use cell phone during exam
