Get in, get examined, and get back to work — on your schedule. Our two Oregon locations give you the ability to schedule your physical examination in advance, avoiding long waits or wasted time. Simply choose a location, book your appointment, and we’ll handle the rest.

Exams take approximately 20 minutes, and we ask that you arrive 15 minutes early to ensure a smooth check-in process. Be sure to review our Physical Exam Checklist before scheduling.

DOT Physical 
Exam Checklist

Everyone needs to:
  • Bring a state-issued photo ID. Best option is your driver’s license.
  • Be prepared to provide a urine sample to test for kidney functions and protein levels
You may also need to bring:
  • Glasses or contact lenses, if you wear them for driving or work
  • Hearing aid(s), if you use them for driving or work
  • Medical Release documentation and/or lab results from your treating physician for any serious medical conditions (diabetes, stroke, heart disease, cancer).
  • CPAP readout or chip if you use one for sleep apnea
  • Medication list
  • Federal Waiver, SPE Certificate, Exemption Certificate
Please make sure you do not:
  • Chew or use tobacco directly before or during the exam
  • Drink excessive beverages containing caffeine before the exam
  • Use cell phone during exam
